Your physician has scheduled you for an appointment. It is important for you to be prepared for this examination.

• All examinations require you to follow a special diet.
• You may have to alter your medication schedule.
• Many examinations require you to use a bowel prep kit.
• You will need to fill out medical and insurance information forms
• You may need to have an anesthesia consent form filled out ahead of time.
• You must have a ride home with a responsible party for all procedures.

Failure to follow these procedures may cause your appointment to be canceled.
